Purpose:

Used for surveys requiring more precise sentiment measurement, such as satisfaction levels or agreement intensity.

Limit:

Scale range typically 1–5 or 1–10.

Validation / Special Configuration:

  • Single choice per row.
  • Can display numeric or descriptive labels.

Use Case:

Used for department-wise or service-level rating surveys.

  • Question: Rate your satisfaction with the following healthcare aspects.
  • Help Text: 1 = Very Dissatisfied, 5 = Very Satisfied
  • Instruction: Choose one number.
  • Scale Minimum Number: 1
  • Scale Maximum Number: 10
