Limit:
Scale range typically 1–5 or 1–10.
Validation / Special Configuration:
- Single choice per row.
- Can display numeric or descriptive labels.
Use Case:
Used for department-wise or service-level rating surveys.

- Question: Rate your satisfaction with the following healthcare aspects.
- Help Text: 1 = Very Dissatisfied, 5 = Very Satisfied
- Instruction: Choose one number.
