Checkbox
Shows individual checkbox options. Users can select multiple independent choices.
Purpose:
Good for toggling features, agreeing to terms, preferences, or selecting multiple properties.
Limit:
Multiple options can be configured; typically limited to 10–15 choices.
Validation / Special Configuration:
- Can be marked mandatory for consent before proceeding.
- Supports single selections.
- Option to add rich text or hyperlinks (e.g., Terms of Service).
Use Case:
Used to confirm agreement with healthcare terms, privacy policy, or consent for data usage.
Question Configuration Example:
- Question: Which of the following health conditions have you experienced in the past year?
- Help Text: You may select more than one condition.
- Instruction: Tick all applicable options such as diabetes, hypertension, asthma, etc.
Note:
If the creator or admin activates “Hide Number”, the checkbox will align inline with the question text.
If “Hide Number” is not active, the checkbox will appear below the question, after the instruction text.
